FIRST TEE – NORTH FLORIDA “ULTIMATE GAME CHANGER RAFFLE” 
OFFICIAL RAFFLE RULES & DISCLOSURES
The Ultimate Game Changer Raffle (“Raffle”) is sponsored by First Tee – North Florida, a 501(c)(3) organization located at 101 East Town Place, Suite 100 St. Augustine, FL 32092 (“Sponsor”).
1. Eligibility:
The Raffle is open to Florida Residents who are 18 years of age or older at the time of entry. Full-time employees of the Sponsor are not eligible to win.
2. Entry Period:
Entries will be accepted beginning on Thursday June 19, 2025, at 12:01pm EST and ending on Wednesday August 27, 2025, at 6:00pm EST (the “Entry Period”). 
3. How to Enter:
A suggested donation of $100 per entry may be made at https://secure.qgiv.com/event/ultimategamechangerraffle/donate/. Donations can also be made in person at 101 East Town Place, Suite 100 St. Augustine, FL 32092. Each donation of $100 will result in one entry to the Raffle. There are no limits to how many entries may be entered for a chance to win for an individual or company making a suggested donation. There will be no minimum or maximum entries for the Raffle to take place. No purchase or donation is necessary to enter to win. Participants may enter the Raffle without making a suggested $100 donation. A donation will not increase your chances of winning. For participants choosing not to make a suggested donation, they may fill out raffle ticket request form, free of any charges, in-person at our office located at 101 East Town Place, Suite 100 St. Augustine, FL 32092 on Wednesday July 16, 2025, between the hours of 10:00am EST and 3:00pm EST or on Wednesday August 20, 2025 between the hours of 10:00am EST and 3:00pm EST for a chance to win. 
4. Prize Description:
One (1) Grand Prize Winner will receive a travel package with an approximate retail value (“ARV”) of $15,000. The winner may choose one (1) of thirteen (13) curated trip packages provided by the Sponsor or its designated travel partner. Travel packages are listed on https://firstteenorthflorida.org/utimate-game-changer-raffle/. Each trip includes accommodations and experiences as described in the prize catalog, which will be provided to the winner upon notification.
The winner must select a package from the available options by August 15, 2025. Travel must be booked within one (1) year of winner selection. Trips are non-transferable, non-refundable, and may not be redeemed for cash. Some blackout dates and restrictions may apply. Airfare and certain expenses may be included depending on the selected trip; full details will be outlined in the prize catalog. Prizes will be purchased using First Tee – North Florida funds.
5. Winner Selection and Notification:
The winning entry will be selected at random from all eligible entries received during the Entry Period. The drawing will be held on Wednesday August 27, 2025, at 6:30pm at 101 East Town Place, Suite 100, St. Augustine, FL 32092 (all entries will be notified if location changes). The potential winner will be notified by email and/or telephone within five (5) business days following the drawing. If the potential winner cannot be contacted, fails to respond within seven (7) days, or is found to be ineligible, an alternate winner may be selected.
6. Odds:
Odds of winning depend on the number of eligible entries received.
7. Taxes:
All federal, state, and local taxes associated with the prize are the sole responsibility of the winner. The winner may be required to complete an IRS Form W-9 and may receive an IRS Form 1099 for the value of the prize.
8. Publicity:
By accepting the prize, the winner grants the Sponsor the right to use their name, likeness, city and state of residence, and prize details for promotional purposes without further compensation or notice, unless prohibited by law.
9. General Conditions:
Sponsor is not responsible for entries that are lost, late, misdirected, damaged, incomplete, or illegible.
10. Limitation of Liability:
By participating in the Raffle, all entrants waive and release any and all claims, now known or hereafter known, against the Sponsor, and its officers, directors, managers, employees, agents, successors, and assigns (collectively, “Releasees”), on account of injury, disability, death, or property damage arising out of or attributable to the entrant’s participation in any activity related to any prize related to the Raffle.   All entrants covenant not to make or bring any such claim against the Sponsor or any other Releasee, and forever release and discharge the Sponsor and all other Releasees from liability under such claims.
